The telephone company may make changes to its facilities, equipment,
operations or procedures that could affect the operation of the equipment.
If this occurs, the telephone company will provide sufficient advance
notice for you to make any modifications necessary to maintain
uninterrupted service.
If you experience any difficulties with the cradle/modem, please contact
the VQ OrthoCare Patient Care Department at 800.452.7993 for repair or
warranty information. DO NOT open the cradle or make any repairs to it
yourself. If the equipment is causing harm to the telephone network, the
telephone company may request that you discontinue use of the cradle/
modem until the situation is resolved.
This equipment must not be used on party lines. Connection to party
line service is subject to state tariffs. Contact the state Public Utility
Commission, Public Service Commission or Corporation Commission
for information.
If your home has specially-wired alarm equipment connected to the
telephone line, make sure that use of the modem does not disable your
alarm equipment. If you have questions about what will disable your
alarm equipment, consult your telephone company or qualified installer.
