Welcome

This guide will help you start monitoring using the

VistaPoint

application and the VitalPatch

biosensor.

• The VitalPatchbiosensor (referred to as “VitalPatch”

in this document) is an adhesive device worn on the

chest to measure and record vital signs. Refer to the

VitalPatch Instructions for Use for detailed clinical

instructions.

• The VistaPointapplication collects and uploads vital

signs from the VitalPatch and approved third-party

devices to your care team. VistaPoint runs on the

portable VistaTablet

™

or VistaPhone

™

device.

In addition to this guide, follow all instructions provided

by your care team.

Indications for Use VistaPoint is a software Graphical User Interface intended for use by healthcare professionals to display physiological data collected by the Vista Solution wireless remote monitoring system in home and healthcare settings. VistaPoint is a secondary, adjunct patient monitor and is not intended to replace existing standard-of-care patient monitoring practices. Contraindications (When not to use) Do not use the VistaPoint application as a standalone diagnostic monitor.

Section 1: Setting Up VistaTablet or VistaPhone Device Monitoring requires that you use the VistaTablet or VistaPhone device provided by your care team. Keep the device near you at all times during the monitoring period. Whenever possible, keep the device plugged into the charger (for example, when sleeping). • • The VistaTablet (see figure to the right) has a large, horizontal screen and may be provided with a protective carrying case. Do not remove VistaTablet from the carrying case.

Charging the Device Use only the provided cable and charger to charge the VistaTablet or VistaPhone. Keep the device plugged into the charger. You can use the device while it is charging. • • VistaTablet uses a micro-USB charging cable. Use caution when inserting the cable. This cable can only be inserted one way. If the cable does not insert into the charging port, rotate the cable and insert again.

Returning the Device After your monitoring period is complete, return the monitoring equipment to your care provider as soon as possible. Failure to return may result in additional charges for monitoring services. The monitoring equipment includes the following: • • • • VistaTablet or VistaPhone USB charger and charging cable Unused VitalPatch biosensors Optional devices for measuring oxygen saturation, blood pressure, or weight.

Section 3: Changing Patient Name and Device Name The VistaTablet and VistaPhone display the following information on the dashboard. The location of these items will vary by device type and configuration. Refer to the figure on the right. • Patient Name. This appears in either the lower-left corner or upper-left corner of the dashboard. You must have a PIN code to change the patient name. DO NOT change the patient name unless directed by your care team. • Device Name.

Section 4: Removing Previous VitalPatch Before applying a new VitalPatch, use the following steps to remove a previously applied VitalPatch (if necessary). 1. Disconnect from VistaPoint IMPORTANT: If the VitalPatch is still connected to VistaPoint, disconnect the patch before removing. Disconnect Menu Tap “Disconnect Patch” from the Menu Disconnect Patch To disconnect, tap Menu and then tap Disconnect Patch. See Section 10 on page 17 for more information. 2.

Section 5: Applying the VitalPatch VitalPatch must be placed on the PRIMARY location on the upper left chest. DO NOT use the SECONDARY location. See below for more details on the location. 1. Remove VitalPatch from Pouch Make sure your hands are clean and dry. Tear open the pouch at the notch and remove VitalPatch. Note: Wait a minimum of 2 minutes before powering-on the VitalPatch. Section 7: Applying the Adhesive Overlay Your healthcare provider may recommend that you apply an optional adhesive overlay to help the VitalPatch stay adhered to the skin. Use only the provided adhesive overlays. If you are not using an Adhesive Overlay, skip to Section 8 on page 11 to pair your new VitalPatch with VistaPoint. Note: DO NOT use other adhesives, overlays or tape. 1.

Section 8: Pairing VitalPatch with VistaPoint After applying the VitalPatch biosensor to the chest, you must pair VitalPatch with VistaPoint. 1. Begin the Pairing Process Begin Pairing Unlock the VistaTablet or VistaPhone (see Section 1 on page 3). Tap Connect VitalPatch from the VistaPoint dashboard. Tap Connect VitalPatch Note: If a previous VitalPatch is still paired with VistaPoint, disconnect it before continuing (see Section 10 on page 17). Guided Help 2.

4. Choose Pacemaker Status The pacemaker status screen may appear during the pairing process. The screen will appear only once per patient (will not appear when replacing a VitalPatch). Choose one of the following: • • Select YES if you have a pacemaker. Select NO if you do not have a pacemaker. If you do not know, please ask your doctor. 7. Calibrate Body Temperature If your configuration supports automatic body temperature (requires VitalPatch firmware release 3.2.0.0 or later), a Temperature Calibration screen will appear, and you will be prompted to calibrate the body temperature. If the Temperature Calibration screen does not appear, the VistaPoint dashboard will be displayed after you perform the posture calibration. Use the VistaPoint Application When pairing is complete, you will be shown the VistaPoint dashboard. Do the following, as directed by your care team: • Keep the device powered on and charged. Keep the device near you as much as possible (less than 30 feet) to ensure constant monitoring. • Use the dashboard to observe vital signs and monitor VitalPatch battery life. • If directed by your care team, take oxygen saturation, blood pressure, or weight readings.

Dashboard Features • Live ECG (not displayed if a pacemaker is indicated). Section 10: When Monitoring is Complete 1. Disconnect VitalPatch in VistaPoint When monitoring is complete, select Disconnect VitalPatch from the menu. Select one of the following options: • Disconnect VitalPatch: Choose this option if you will be applying a new VitalPatch. • Discharge Patient: Choose this option if you will NOT be applying a new VitalPatch. All devices will be disconnected. Use as directed by your care team. 2. Remove VitalPatch To remove the VitalPatch, see Section 4 on page 7.

Section 12: Making a Video Call or Appointment Your care team may offer the ability to make care-related video calls or appointments using your VistaPhone or VistaTablet. This feature is not available on all devices. To start a video call, tap Menu and then tap Applications. Select the video call application. For more information, refer to Zoom Instructions for Use (IFU-16).

Section 15: Entering Body Temperature Manually Your care team may direct you to enter body temperature manually on a regular schedule (for example, one or more times per day). Entering Body Temperature Manually To enter body temperature manually: Tap Body Temperature 1. Measure temperature with a clinical-grade thermometer. 2. Tap the Body Temperature tile in the dashboard (if using VistaPhone with simplified display, tap Take a Measurement button first). 3. Tap the units (Celsius °C or Fahrenheit °F).

Section 17: Entering Blood Pressure Manually Your care team may direct you to enter a blood pressure reading manually on a regular schedule (for example, one or more times per day). This procedure is for manual entry only. If you are performing automatic entry with a paired Bluetooth device, refer to Section 27 on page 35. 1. Use a blood pressure device to measure your blood pressure. 2. Tap Take BP in the Blood Pressure tile (or tap Take a Measurement button on VistaPhone and then Take BP). 3.

Section 19: “VitalPatch Off Body” Message If the screen displays the message “VitalPatch Off Body,” this indicates that the VitalPatch no longer detects proper skin contact. See figure to the right. • If removal was intentional, tap Disconnect Patch to end the monitoring session. • If removal was not intentional, inspect the VitalPatch for adhesive peel-off or lifting on the edges. If the adhesive looks intact, press the VitalPatch down against the skin to re-adhere and tap Wait for Reconnect.

Section 20: Other VitalPatch Error Messages The screen may display error messages related to the VitalPatch. These are routine events. If the event is not listed here, or if the device is not operating correctly, refer to “Troubleshooting” in Section 21 on page 23. Message: “VitalPatch Not Detected” Indicates that the VitalPatch is out of range, has lost Bluetooth connection, or has expired. • If disconnection was intentional or VitalPatch has expired, tap Disconnect Patch to end the monitoring session.

Section 21: Device Troubleshooting Wireless Connection Error Messages See figure to the right. The VistaTablet or VistaPhone may display the following error messages that indicate a problem with your wireless connection: Connection Error Messages Cloud Service Down The cloud service is currently down. This problem will be resolved soon. OK Message: Cloud Service Down If you see this message, it indicates that the cloud service is not operational at this time. Tap OK to dismiss the message.

Dashboard Stops Responding If vital signs do not appear to be updating or the screen becomes unresponsive (for example, when pairing devices), you can refresh the current view by dragging your finger across the display and then releasing. Section 22: Oxygen Saturation (Berry BM1000 Fingertip) The Berry BM1000 is an optional pulse oximeter device that measures blood oxygen saturation (%SpO2) and uploads readings automatically using Bluetooth. The device is clipped onto the fingertip to take a reading. Berry BM1000 Fingertip Pulse Oximeter Your care team may direct you to take an oxygen saturation reading with the Berry BM1000A on a regular schedule (for example, one or more times per day).

ID of the Berry device when it appears. If more than one Bluetooth ID is shown, please ensure there are no other Berry devices powered on (but not paired with a VistaTablet) nearby. f. VistaPoint displays “Please wait connecting…” and then displays the current oxygen saturation value on the dashboard. If you see a message asking you to confirm the device pairing, tap Confirm. g. If the monitoring session is complete, tap Disconnect in the Oxygen Saturation tile.

Section 23: Oxygen Saturation (Berry BM2000A Wrist) The Berry BM2000A is an optional pulse oximeter device that measures blood oxygen saturation (%SpO2) and uploads readings automatically using Bluetooth. The device is worn on the wrist and uses a separate fingertip sensor.

Section 24: Oxygen Saturation (Masimo MightySat™ Fingertip) The Masimo MightySat™ is an optional pulse oximeter device that measures blood oxygen saturation (%SpO2) and uploads readings automatically using Bluetooth. It is clipped onto your fingertip. b. Swipe the touchpad until the About screen is displayed. Tap the touchpad to display the information screen. See figure to the right. The Bluetooth ID is the number under “mac”. Nonin 3150 Pulse Oximeter The Nonin Model 3150 is an optional pulse oximeter device that measures blood oxygen saturation (%SpO2) and uploads readings automatically using Bluetooth. It is worn continuously on the wrist and uses a separate fingertip sensor.

Section 26: Oxygen Saturation (Nonin 3230 Fingertip) Note: If the device has already been paired, go directly to Step 3 “Take a Reading” below. The Nonin Model 3230 is an optional pulse oximeter device that measures blood oxygen saturation (%SpO2) and uploads readings automatically using Bluetooth. It is clipped onto your fingertip. 4. Take a Reading Refer to the A&D Model UA-651 Instruction Manual for cuff application instructions. a. Tap Take BP in the Blood Pressure tile (or Take a Measurement on VistaPhone and then Take BP). b. Tap Use Bluetooth (if not already selected). c. Tap Take Reading.
