ACCEPTANCE
Equipment is provided
to
residents
of
the State of Florida for their use
but
remains tho property
of
FTRI.
It is
the
responsibility
of
the individual using this property
to
care for
it
and
to
return
it
to
FTRl
if
they are no bnger a Florida resident.
The
appllcent
agrees
to
protect the equipment from damege8

If
equipment
Is
abused,

applicant
can
be
held

responsible

THEFTlFlRE
In the event equipment
IS
stolen
or
destroyed, the applicant
will
be required
to
provide a
copy
of
the
pdice/sheriffAire
WOrt
before a replacement
will
be
issued.
If
the loss
is
covered
by
insurance
the
proceeds
from
any claim will
be
forwarded
to
FTRl
at
the
address
below.
LOST
If
the equipment is lost, the applicant must write
to
FTRl
at
the
address
below.
The applicant understands that he/she
will
not
be
immediately issued (loaned)
a
replacement piece
of
equipment.
DAMAGElREPAIR
Should
the
equipment fail, the applicant is responsiMe for contacting FTRI, or a local regional distribution center.

is damaged, the applicant
will

to
repair
or
disassemble.

travel
out

CHANGE
OF
ADDREWMOVING
The
applicant
is
responsible
for
notifying
FTRl
of
any change
in
address
or
telephone
number
within
twenty
(20)
business
days.
If
the applicant
moves
out
of
the State of Fbria, hdshe must contact
FTRl
or
a
local regional distribution center at least
three
(3)
weeks
BEFORE
he/she leaves the State of Florida for instructions on returning equipment.
SALE
OR
LOAN
Because the equipment
is
the
property
of
FTRI,

will
not sell it.
The
applicant will not give
it
or loan
it
to
others.
The applicant understands that
if
the equipment is
sold
or loaned,
the
applicant
can
be
held responsible
for
replacement

FTRI

warranties

merchantability
of
fitness for particular service
in
connection with the transfer
of
this equipment. The user

equipment

responsible for the use and operation of equipment received
by
himher.
DENIAL
Failure
to
comply
with
these Conditions
of
Acceptance
can
result
in
the applicant's being
denied
participation
in
the
Spectalized
Telecommunications Equipment Distribution Program.
Having read the above conditions or having had them explained
to
