Qualifying
is
a
simple
process.
Just
meet
these eligibility requirements:
Re a Florida resident.
Be three years
of
age
or
older to receive the
Volume
Control
Phone
for
the Hearing Impaired
(VCPH),
In-Line Amplifier
(ILA), Audible Ring Signaler
(ARS),
or
Visual Ring
Signaler
(VRS).
Be
five years of age
or
older to receive a Text Telephone
(TTY).
Bc ccrtified as having one of the following impaimients:

Deaf:
You
must
have permanent hearing
loss
and be
unable to recognize speech sounds during telephone
convcrsations with
or
without an amplification device.
Hard
of
Hearing: You must have a permanent hearing
loss
severe enough that you need
to
use
ampliiication
devices to recognize speech sounds during telephone
conversations.
Deaf/Blind:
You
must be deaf and blind or have both a
permanent hearing and visual impaimlent.
Speech Impaired:
You
must have a permanem: speech
impairment
so
that you cannot use a standard telephone.

the specialized
equipment
is
easy.
Just
follow
these three steps:

Fill out the attached FTRl equipment application. (See the
instructions that follow.)

Return your abplication.
Florida residents have choices as to how they receive their
equipment. They may visit one of
FTRl’s
Regional Distribution
Centers (RDCs),
or
they can have their equipment application
processed through
FTRI
by mail. They also
can
request
to
have
their equipment shipped to their certifier,
if
appropriate.

FTRI
will process
your
application and ship your equipment
to
you through
UPS.
Clients who wish can have their equipment sent
to
their
certifier. Please note this request
on
your application under
shipping address.
Mail the original (white) copy of the application and the
RDC (yellow) copy to FTRI at
