Note: Block formatting combines multiple images and does not allow each signature print separately.

7. Do you have a digital copy of your logo? (Skip if not applicable) YES NO

If yes, please email to Digitalimg@troygroup.com and include the name of your company in the subject line. If no,

please attach a black and white copy to this order form

8. Please provide any additional information or special instructions (e.g. additional job numbers, transfer details, etc.).

If yes, please provide the correct SERIAL number or JOB number you wish to duplicate or transfer images from. If you

need to duplicate multiple jobs, you can provide additional job numbers in step 8. If you need to transfer certain

images in exclusion to others, please indicate which images need transferred in step 8.

If yes, please provide the SERIAL number or JOB number of your most recent TROY digital image device (DIMM, CARD,

USB or other). Do NOT use the printer serial number. The device serial and job number can be found on a label

attached directly to the device and on the "Digital Image Data Sheet" which was shipped with your current device.
