Exhibit A
SCOPE OF SERVICES
Lease of fifty (50) new multi-functional copiers and all associated accessories, service,
maintenance, and supplies (excluding paper), as specified per all terms and conditions of the
Request for Proposal (RFP) for Multi-Functional Copiers and Copier Maintenance (RFP#029-
13) and the subsequent proposal response from Pacific Office Automation. The original Request
for proposal (RFP) documents and the proposal response from Pacific Office Automation will
serve as a continuing part of Exhibit A. The detailed list of copiers and accessories are attached
and incorporated herein.
Pacific Office Automation shall:
Provide adequately trained personnel to perform the copier repair/maintenance services.
Provide NEW multi-functional equipment (copiers and accessories). NEW equipment is
defined as units that have been assembled for the first time, with all new parts and
accessories. New equipment shall be delivered with no more than one-thousand (1000)
testlset-up copies on the meter.
Install all copiers that are a result of this RFP. Upon the expiration or termination of any
agreement, the vendor shall remove said copiers at its sole expense. In addition, if any
copier(s) would need to be relocated for any reason, the vendor will also provide that
service at no expense to the County.
Participate in bi-annual account reviews with Purchasing Division staff.
Provide one (1) quarterly invoice, billed in arrears, for machines and actual copies made.
Black & White copies are billed at $00055/copy and Color copies are billed at
$00500/copy. Standard fiscal quarters apply
March, June, September, and December.
Upon the expiration or termination of any agreement, all hard-drives will be removed by
the vendor and retained by Douglas County at no additional expense to the County.
Ensure that all personnel performing services under this Agreement will perform in a
professional manner and will comply with all of the County's applicable security and
safety regulations
Should the County be dissatisfied with the performance of any vendor
personnel,
the
County will
provide written
notification to
the vendor of the
specific non-performance.
The successful vendor and associated personnel must pass a Federal Fingerprint
Background check prior to the start of work and access to County facilities.
The
fingerprint/background checks will be completed by Douglas County. Douglas County
reserves the right to deny access to any employee on County property, who is identified
as a potential threat to the health, safety, security, general well-being, or operational
mission of the County and/or its population.
Should access be denied, the successful
vendor will replace the employee with a suitable substitute, as approved by the County.
Provide a small desktop copier to be used for approximately one-week (in August) during
the Douglas County Fair and a mid-size stand-alone copier to be used for approximately
two-months (in the fall) during the Douglas County
Board
of Equalization
(BOE)
Hearings.
