Record Start of Test Information
Once your device is registered and the start
date is recorded on the online Data Form, you
can begin your test.


AFTER 2-4 DAYS
End the Test
Snugly place the cap
back on the vial.

Leave the test in
place for 2- 4 days
(48 -96 hours).

Begin the Test
Twist and remove the cap
from your vial and set vial in
the selected test location.
Record End Date
Scan QR code at top of page or visit
accustarlabs.com/ls
to complete the end
of test information
on the Data Form.

Return Device
to Laboratory
Using FedEx or UPS (USPS not recommended),
return the device to the lab. We MUST receive
your device within 7 days of the end of the test.
Getting Your Report
You will receive your report via email the next business day
after your device arrives at the lab.
You may also view your report at accustarlabs.com,
where you will nd additional information and resources
regarding your report.
• Scan the QR code with your mobile phone or visit
accustarlabs.com/ls to register device and ll out
Data Form.
• Ensure that testing guidelines are followed. Scan the QR code
for more information.
• DO NOT open test device until you are ready to start your test.
