Primary specialty
Primary specialty for your practice.
You can search for a primary specialty by using the
wildcard feature. Begin typing the first three letters in
the field to see matching specialties.

Secondary specialty
Secondary specialty for your practice, if applicable.
You can search for a secondary specialty by using the
wildcard feature. Begin typing the first three letters in
the field to see matching specialties.

User type
Select an option to indicate this user's type.
