The Contractor shall submit a written report upon completion of testing to the CPM II or
authorized designee that details the work performed and the status of the systems
worked on. The written report shall include each item required to be checked and
condition thereof (i.e., satisfactory, unsatisfactory), totals of fluid added, or drained
and/or replaced, gauge and meter readings. Written reports shall include any potential
problems that the Contractor becomes aware of and include the most economical repair
alternatives.
IV.
LICENSE/PERMIT/CERTIFICATION REQUIREMENTS
Contractor shall possess a valid C-10 Electrical Contractor license and be registered
with the California Contractors State License Board.
The Contractor shall have a minimum of two (2) service technicians for each institution
area who have been trained on the equipment specified herein. The Contractor shall
provide the CDCR with proof of the required training and/or experience for each service
technician providing service under this agreement.
The Contractor shall certify that technicians have ready access to repair manuals,
schematic diagrams, and to manufacturer’s replacement parts for all equipment listed in
Exhibit A-1, Equipment List.
The Contractor shall meet the qualifications by one of the following patterns:
I – Certificate and Statement of Qualifications
The Contractor shall complete Attachment 2, Service Technician Training and attach
Certificate(s) of Training i.e.; trade school or manufacturer for generator maintenance
and repair. Attachment 2, Service Technician Training shall include the following:
• Technician name
• List of all equipment certifications, a copy of each equipment repair certificate
• List the firm, contact information and dates of training
• Specifically list the equipment training that is similar to the equipment specified in
Exhibit A-1, Equipment List
OR
II – Work Experience
The Contractor shall list three (3) professional references on Attachment 3, Service
Technician References for each service technician who will be providing services as
specified. The qualifying experience shall have been performed within the last
twenty-four (24) months consecutively prior to date of bid opening in maintaining
and repairing equipment specified herein. The references shall include the following:
• Technicians name and contact information
• List the firm, contact information and dates of service
