ID Band sizes are marked by the following color
bars for easier identifi cation.
Use colored wristbands with the alert
pre-printed on them (e.g., “fall risk”).
Remove any “social cause”
wristbands from patients
to avoid confusion.
Remove patient wristbands that
were applied at another facility.
Place bands on patients at
admission, when their condition
changes or when new information is
received during the patient’s stay.
Explain to patients the meaning behind
colored wristbands.
Color-coordinate the patient chart,
white board, care plan, door signage
and any stickers with the same color
as the wristband.
Train staff to verify patient color-
coded wristbands upon assessment,
at handoff of care and during transfers
within the facility.
