This form is designed to record information for four weeks. Fill in the dates for which this log will be used in the first line of the table below. For each item listed at the left, fill in your initials under each date that you perform the item. Note: Follow your standard laboratory safety practices when cleaning or maintaining the system. Do not remove the analyzer cover under any circumstances.
