This attachment contains an example of the Itron ChoiceConnect® Antenna and Coax Sweep Test Form. Itron provides this form to
the supplier, so that the supplier can determine their fixed fee, lump sum, and all inclusive pricing needed to complete the expected
number of network device installations.
