path. Reasons could include, failed network path, lack of login, or disabled
ports.

The number of target logins is limited to 32 per controller. Verify that the limit
has not been reached by executing show tgt_stats on each target, adding
up the total number of sessions up.
iSCSI conguration was

chassis swap out
All iSCSI information is tied to the serial number of the device. Swapping
chassis results in a change in the serial number.

connectivity with

Duplicate IP address on the network.
Failure of network path.
Use of DHCP for server IP addresses.
Reinserted controller fails
to start up
Reinsertion of a controller within ten seconds of removal may lead to the
failure of the other controller. Allow more than ten seconds between removal
and reinsertion.
