when cellular service is available. **5Star Services only function with Lively Wearable2
when Lively Wearable2 is paired and connected by Bluetooth to your compatible
smartphone (not included) and your smartphone is charged, turned on, and has network
access. 5Star Service tracks an approximate location of the device when the device is
turned on and connected to the network. Fall Detection is an optional feature and may
not always accurately detect a fall. For Lively Mobile Plus and Lively Wearable2, Optional
Fall Detection functions only when the device is used with specially designed lanyards
we provide to you. The Lively Mobile Plus and Lively Wearable2 are IPX7 waterproof
rated for immersion in up to 3.3 feet of water for up to 30 minutes. Urgent Care, provided
by FONEMED®, is not a substitute for dialing 9-1-1 and should not be used in a case
of emergency. GreatCall is not a healthcare provider. Seek the advice of your physician
if you have any questions about medical treatment. FONEMED’s registered nurses and
contracted physicians through MDLIVE (the “Providers”) offer advice regarding healthcare
decisions, may prescribe certain medications and make diagnoses. We are not liable for
any act or omission, including negligence, of any FONEMED employee or contractor.
Handset and device replacement eligibility begins at 60 days from program enrollment
date. Limit one replacement per 12 month period.
