Apply the following settings to the Default Form Display Settings section:
13. Select the User Interface dropdown and choose the Drop-down
list option.
14. Enter a Label of: Age Group:
15. Enter a Description of: Please select your age group.
16. Leave the CSS Class field empty.
17. Leave the CSS Style field empty.
18. Leave the Legend field empty.
19. Select the Options Generator dropdown and choose the Use
options option.
20. Enter the following text into the Options field:
none|Select your age group...
1|0 to 19
2|20 to 29
3|30 to 39
4|40 to 49
5|50 to 59
6|60 to 69
7|70+
21. Select the Sort field and choose the No sorting option.
