Primary Shipping Contact First name The first name of the primary
contact who will be responsible for
receiving the dispatched part.
Last name The last name of the primary
contact who will be responsible for
receiving the dispatched part.
Phone number The phone number of the primary
contact who will be responsible for
receiving the dispatched part.
Email address The email address of the primary
contact who will be responsible for
receiving the dispatched part.
Secondary Shipping Contact First name The first name of the secondary
contact who will be responsible for
receiving the dispatched part.
Last name The last name of the secondary
contact who will be responsible for
receiving the dispatched part.
Phone number The phone number of the secondary
contact who will be responsible for
receiving the dispatched part.
Email address The email address of the secondary
contact who will be responsible for

Shipping Address Time Zone The time zone of the primary or
secondary contact.
Preferred contact hours The preferred hours when Technical
Support can contact the person
who is responsible for receiving the
dispatched part, if there are any
issues.
Location Select the country or region.
Shipping Address The address where a replacement
component must be dispatched.
