Benefits coverage and vacation during long term disability leave
Upon approval from our benefits provider, you may continue to participate in the group
extended health and dental plans while you are away from work and in receipt of Long Term
Disability benefits provided that you continue to pay your share of the premiums. Please note
that some insurance premiums may be waived by the benefit provider during receipt of LTD
benefits. After 1 year, and subject to continued approval by our benefits provider, we reserve
the right to make the continuation of your extended health and dental benefits contingent upon
your payment of all premium costs (i.e. both the employer and employee portions). Post-dated
cheques will be required for each benefit year to be covered during your leave while on LTD.

Please also note that vacation does not continue to accrue while on LTD.
Benefits at end of your employment
Once you are no longer employed by Bradley Smoker, you are eligible to continue coverage for
medical and dental coverage only, for an additional 18 months under COBRA. Upon
termination, you will receive a COBRA rights notice. In order for your coverage to be reinstated,
you must submit your COBRA election within 60 days of receiving this notice. To maintain
coverage, premiums must be sent each month. You will be responsible for paying 100% of the
premium rate. Please note that an additional 2% administrative charge may apply to the
premium rate.
You may also be eligible to convert your Life Insurance to an individual policy within 31 days of
your last day of employment with Bradley Smoker, without needing to provide medical
evidence. For further information please review the current benefit handbook or contact the
payroll and benefits administrator.

Mud Bay Supply is our specialty barbeque grill and smoker store (located at our Delta, BC
building) which specializes in Bradley Smokers (of course) but also carries a range of other
products, including grills, BBQ tools, bisquettes, cookbooks and more! As an employee of
Bradley Smoker, you are entitled to a 15 - 20% discount. Following are the staff purchase
guidelines:
Only Mud Bay employees are permitted to process purchase transactions for employees
and their families; i.e. you may not do so on your own
You may not purchase items that have not previously been publicly offered to customers
(e.g. brand new, unlaunched products)
