Employee assistance plan (EAP)
As part of benefits offerings, we provide an EAP to employees under the Unum plan. An EAP is a
comprehensive and confidential source of information, education, and consultation on a wide
range of issues that you may need assistance with (including but not limited to child care,
education, parenting, family and work issues, addiction problems, financial and legal
concerns). For further information and to access assistance, go to www.lifebalance.net (ID and

Entitlement waiting period, enrollment and coverage changes
Following 90 days of continuous service, you will be eligible to take part in all benefit
components. As a new employee, if you wish to enroll in the medical and dental plans, you
must do so immediately upon becoming entitled. If you choose to opt out of medical and dental
at this time but wish to enroll later, you may do so during our annual open enrollment period
effective January first annually.
Open enrollment is a once-a-year opportunity for you to make changes to your insurance
and/or beneficiary information and to enroll or re-enroll in the plan. As the benefits plan renews
on the 1
st
of every February, effective January 1
st
, you have 30 days to make changes to your
elected benefit options. You may only make changes to your benefits during this 30 days period
or when you have experienced a specific qualifying event.
A qualifying life event occurs is a change in family status or involuntary loss of coverage, such
as one of the following examples:
Change in marital status
Birth or adoption of a child
Death of a covered dependent
Loss of eligibility status by a covered dependent
Change in employment status that affects eligibility for coverage
Losing or gaining healthcare coverage eligibility under Medicare or Medicaid
Change in residence to a location outside of a healthcare plan’s service area
If any of the above should occur, notify the Payroll & Benefits Administrator within 31 days of
the event and complete the appropriate forms to make changes to your coverage.
Pre-tax premiums
In order to provide you with a tax benefit (i.e. less income tax paid), premiums will be taken out
pre-tax.
Doctor coverage and making claims
Prior to seeing your regular doctor for medical services, in order to receive the highest benefit
level, be sure s/he is a primary care physician. To do so, visit their website: www.aetna.com,
click Menu then Find a Doctor. From there, scroll down and click Find an Aetna Doctor. Enter
your search criteria, then select Medical, and click Search.
Claim information will be provided to you along with your benefit package.
