In the case of Life, AD&D and LTD, employees pay the premiums so that actual payments will
be tax-free. In the event that we paid the premiums on your behalf, your payments would be
subject to tax thus significantly impacting the amount of money paid to you if/when needed.
Entitlement waiting period
Following three months of continuous service, you will be eligible to take part in all benefit
components.
Making claims
Claim information will be provided to you along with your benefit package.
Benefits coverage and vacation during long term disability leave
Upon approval from our benefits provider, you may continue to participate in the group
extended health and dental plans while you are away from work and in receipt of Long Term
Disability benefits provided that you continue to pay your share of the premiums. Please note
that some insurance premiums may be waived by the benefit provider during receipt of LTD
benefits. During this period, we will also continue to cover a portion of your Medical Service
Plan (MSP) premiums; however, please keep in mind that this portion is a taxable benefit and
must be reported on your tax return.
After 1 year, and subject to continued approval by our benefits provider, we reserve the right to
make the continuation of your extended health and dental benefits contingent upon your
payment of all premium costs (i.e. both the employer and employee portions). At this point,
MSP will also cease under the Bradley group plan and you will be solely responsible to pay your
premiums as billed by MSP.
Post-dated cheques will be required for each benefit year to be covered during your leave while
on LTD. Please speak with the Payroll and Benefits Administrator for additional details.
Please also note that vacation does not continue to accrue while on LTD.
Benefits at end of your employment
If your employment ends either voluntarily or involuntarily with cause, you will cease to
participate in our group benefits plan and will not be entitled to any further benefits. Unless
agreed otherwise, if your employment ends involuntarily without cause, following the statutory
notice period, you will cease to participate in the group benefits plan and will not be entitled to
any further benefits; within the statutory notice period and with approval from our benefits
provider, all or part of the benefits may continue for the duration of the notice period.
Upon cessation of benefits in either case, you will be responsible for obtaining personal benefit
plans to replace any or all benefits provided by Bradley Smoker.
Please note that you may be eligible to convert your Life Insurance to an individual policy within
31 days of your last day of employment with Bradley Smoker, without needing to provide
medical evidence. For further information please review the current benefit handbook or
