Complete a WorkSafe Application for Compensation and Report of Injury or Occupational
Disease (Form 6) with your manager within 48 hours; if you don’t do so, this could result in
the loss of WorkSafe BC compensation benefits
In order for a claim to be established, your manager must send a completed WorkSafe
Employer’s Report of Injury or Occupational Disease (Form 7) and an Accident Investigation
Report to WorkSafe BC within 3 days of your injury or illness
Once a claim has been established, continued correspondence regarding your claim will be
directly between you and WorkSafe BC
Note: work is considered “missed” if you miss your next full day of work. If you leave work on
the date of your injury, it is not considered as work missed by WorkSafe.
Injury or illness employee recordkeeping and reporting procedures (OSHA Claims)
During your employment, you are covered under the Occupational Safety & Health Act for
Injuries and illness that occur as a result of your job.
If an injury or illness occurs, observe the following procedures.
Report your injury or illness immediately to the First Aid Attendant and your manager
regardless of the severity
If you are a witness to an injury or illness and are not a designated First Aid Attendant,
please help locate a First Aid Attendant
If the injury or illness is life threatening, call 911 and the First Aid Attendant will administer
first aid immediately to stabilize the condition
If necessary, the manager or First Aid Attendant will arrange transportation to the hospital
emergency room
If the illness or injury resulted in a fatality, or hospitalization, amputation or loss of an eye,
the manager must report it to OSHA at 1-800-321-6742 or call the closest Area Office
during regular business hours; work-related fatalities must be reported within 8 hours and
work-related hospitalizations, amputations or losses of an eye must be reported within 24
hours
Based on the criteria below, if the illness or injury needs to be recorded, the manager must
record it on the Log of Work-Related Injuries and Illness (OSHA’s Form 300) and complete
the Summary of Work-Related Injuries and Illness (OSHA’s Form 300A) and the Injury and
Illness Incident Report (OSHA’s Form 301). The Form 300A must be posted (excluding
personal private information) in the workplace the following year from February 1
st
to April
30
th
. The Form 300 and Form 300A must be retained for 5 years.
Illness and injuries that result in the following must be recorded:
o death
o loss of consciousness
o days away from work
o restricted work activity or job transfer
o medical treatment beyond first aid
