Voice/Data Terminal Equipment Assignments

Use Table E to list the system users and their individual equipment needs. Blank columns are

provided so that you can fill in the name, extension number, application (voice and/or data), type

of terminal equipment, and adjunct(s) required for each user. A current phone directory and sta-

tion survey may be helpful when filling in this table.
