PRODUCT AND MANUFACTURER SUMMARY
The bidder is required to identify below the system provider intended to be used on this project.
Bids which fail to identify the system provider will be considered non-responsive, and may be
disqualified. Bidders will not be allowed to change suppliers after bid-opening date, except at the
Owner's permission for circumstances as outlined in specification Section 1600.

On acceptance of the proposal for said work, the undersigned hereby agrees to enter into a
Contract and properly secure the performance of same within ten (10) days after being notified
that such Contract is ready for signature, and hereby further agrees to finish and complete said
Work within 120 calendar days. If your company is unable to complete this project in the
aforementioned time frame, the project will be completed within ____ calendar days.
The undersigned will submit the required Performance Bond in the sum of the full amount of the
proposal with surety satisfactory to the Board of County Commissioners, Montgomery County,
Ohio, within ten (10) days (not including Sunday and legal holidays) from the date of
notification of the award.
