Personal Hygiene
It is in the client’s best interests to thoroughly rinse with clean water and dry the skin
underneath the bracelet. Ensure that the client understands that failure to rinse away all
soap may result in a mild skin rash.
Agreement Customization
The following information appears, by default, on the document:
Participant Name Bracelet Serial Number
Agency Modem Serial Number
Agent Name Reporting Schedule
Date Placed on Program
The agreement can be customized to fit how you run your SCRAM Program.
Specifically, you can modify the following:
Upper-left image Additional Hook Up
Participant Name (title) Full Replacement of the SCRAM Bracelet
Agency (title) Full Replacement of the SCRAM Modem
Agent Name (title) Faceplate and Battery Replacement
Date Placed on Program (title) Straps Replacement
Initial Here (title and lines) Participant (title)

In addition, the default agreement is available in Spanish. The agreement includes all the
“do’s and don’ts” in which the client must be aware. From a legal standpoint, you are
laying the foundation with the client and this process will be subject to the scrutiny of the
court if a revocation violation is filed. Refer to the SCRAM Program Participant
Agreement on page 102 for details.
An addendum is also available if the scheduled communication time is changed or if a
new bracelet and/or modem is assigned to the client.
